Payment
Payment is due at the end of each session unless an agreed upon schedule of payment is decided. Electronic payments, cash, and checks are accepted. Credit cards or HSA cards are accepted with a 3% processing fee (credit card fee paid out of pocket).
I am happy to provide rates for evaluations and therapy sessions - please contact me.
Insurance
If you have a PPO plan, your insurance will likely partially cover for out-of-network services. I am currently not in-network with any HMO or PPO plans. I can provide you with a receipt of payment or "superbill" that you can submit to your insurance company for reimbursement of services.
I strongly suggest you check with your insurance to determine your deductible and the percentage of coverage for services. I encourage you to find out about your coverage, and if any preauthorizations are required to evaluate or treat your child.
I am happy to provide any information to help you navigate and better understand insurance. Assessment reports and progress notes may also be provided for you to submit to your insurance company.
